Facilitate self-awareness and engagement of diabetes patients

I worked with the Allscripts Analytics team, the American Diabetes Association, and the Apple Research Kit team to develop the Diabetes Defend application. The project was designed to deliver predictive algorithms incorporating health and socioeconomic data to better understand the progression and consequences of diabetes.

PROJECT GOALS:

Patients gain insights that would optimize self-control and awareness of disease trajectory and compliance of care plans.
Payers and researchers use data in the creation and optimization of predictive models to accurately define those at the highest risk to improve patient engagement, reduce morbidity, and reduce healthcare costs associated with the treatment of diabetes.

Self-awareness among type 2 diabetics and pre-diabetics is low, and behavior change is difficult because patients often do not identify with the risks communicated by healthcare professionals and large scale studies. Patients would use the app to participate in surveys and track data related to medication, diet, exercise, glucose levels, and well-being. This data along with the data from their electronic health record and over 40 million other diabetic patients would be analyzed to set goals and recommendations for the patient to follow. The goal was to increase patient engagement and compliance with treatment plans resulting in improved outcomes.

Bi-directional Data Requirements:

  • Onboarding Survey: Collected medical history and existing conditions such as, neuropathy, retinopathy, renal function, history of stroke, myocardial infarction and medications not documented in the electronic health record

  • Vital Collection: Weight, glucose, blood pressure and heart rate monitoring

  • Activity Tracker: Accelerometer to detect movement

  • Daily & Weekly Checks: Brief survey to collect data about mood and well-being

  • Food Log: Camera app to estimate calorie intake and location data to identify restaurant data

  • Appointment Reminders

  • Third-Party Connections: 23andMe, YMCA Diabetes Prevention Programs, etc.

  • Patient Portal: Connection to patient's electronic health data
